Standard #4 - Evaluation
The registered psychiatric nurse evaluates and assesses progress in the individual's health status in
relation to the goals of the psychiatric nursing care plan.
Rationale:
As a member of the health care team, the registered psychiatric nurse shares responsibility for evaluating
and assessing the effectiveness of interventions so that appropriate changes can be made in the treatment
and nursing care plans.
Audit Factors:
- Identifies the results of nursing actions.
- Compares the result of nursing actions to planned objectives.
- Discusses progress towards goals with the individual (and family).
- Modifies the psychiatric nursing care plan on the basis of progress towards objectives and
effectiveness of intervention.
- Involves the individual in all phases of the problem solving process.
- Records and communicates to appropriate others the evaluation of nursing actions taken.
